What is shoulder impingement?

Shoulder impingement, sometimes called subacromial pain syndrome, means the tissue underneath the top of your shoulder is getting compressed as you raise your arm. The rotator cuff tendons and the bursa sit in a narrow space, and when that space is reduced, they get irritated.

The signature is a painful arc. You usually keep most of your range, but one specific band of motion pinches, often between roughly shoulder height and overhead. Reaching for a seatbelt, pressing overhead, or lying on that side is when you notice it.

The important part is why the space shrank. Most of the time the shoulder blade is not moving or holding position the way it should, or the cuff is not strong enough to keep the joint centered under load. The pinch is a symptom of that, not the cause.

At Rebuild Chiropractic + Strength we test the shoulder blade and cuff together, calm the irritated tissue, then load the shoulder progressively. Loading is usually the part people were told to avoid, and it is usually what fixes it.

What causes shoulder impingement?

Impingement is almost always about the space the tissue has to move through, and what closed it down.

Repetitive overhead work

Painting, pressing, swimming and racket sports load the same narrow space thousands of times until the tissue underneath gets irritated.

Shoulder blade that doesn't cooperate

If the blade does not rotate and tilt as the arm rises, the space closes down and the cuff and bursa take the compression.

A weak rotator cuff

The cuff keeps the ball centered under load. When it fatigues, the joint drifts and the space narrows in exactly the range that hurts.

Training volume that jumped

A sudden increase in overhead pressing or throwing outpaces what the shoulder has adapted to handle.
